Credit Calculation

Credit is calculated based on estimated time-on-task per episode, following ANCC guidelines for enduring materials. Each episode encompasses seven in-game nights:

Core narrative gameplay (clinical scenarios, decision-making)45-75 min
Clinical decision points with embedded assessment15-25 min
Post-episode clinical debrief and knowledge check10-15 min
Evidence-based reference review (linked guidelines)10-15 min
Total Estimated Time per Episode80-130 min

Credit designated in 0.25-credit (15-minute) increments, rounded to nearest quarter hour.

Clinical Education Content (Season 1)

The following clinical domains are integrated into the seven-episode narrative arc, co-developed with clinical faculty consultation:

  • Acute coronary syndromes: troponin interpretation, risk stratification, escalation criteria
  • Volume overload and heart failure with reduced ejection fraction: diuretic management, hemodynamic monitoring
  • Gastrointestinal hemorrhage: upper vs. lower GI bleed workup, transfusion triggers, GI consultation criteria
  • Decompensated cirrhosis: SBP evaluation, hepatorenal physiology, lactulose titration
  • ICU escalation decision-making: recognizing hypotension requiring vasopressors, hypercarbia and hypoxia thresholds
  • Cross-cover and triage prioritization: managing simultaneous competing clinical demands during overnight call
  • Systems-based practice and ethical reasoning: resource allocation in small hospital settings

Sample Learning Objectives (Episode 1)

Upon completion of Episode 1 ("Cradle"), the learner should be able to:

  • Identify the initial evaluation steps for a patient presenting with acute chest pain during an overnight shift
  • Apply evidence-based triage principles to prioritize simultaneous clinical demands
  • Recognize the clinical and psychosocial challenges unique to locum tenens practice
  • Describe the appropriate escalation pathway for a hemodynamically unstable patient in a resource-limited setting
